Plum Creek Medical Group Gets High Marks for Quality and Performance
(Lexington, Neb.; Oct. 28, 2014) – Plum Creek Medical Group, a family medicine health care provider serving Lexington and the surrounding area, received top marks for its quality services in a federal assessment of care provided by accountable care organizations (ACOs).
Plum Creek Medical Group (PCMG), a member of the South East Rural Physicians Alliance (SERPA) ACO, met all quality performance standards set by the Centers for Medicare & Medicaid Services (CMS) and is a partner in the shared savings program, which helps medical providers collaborate to lower costs and improve quality of care for Medicare fee-for-service patients.
“This assessment gives us an opportunity to demonstrate the added value we give patients,” said Jim Naeve, CEO of Plum Creek Medical Group. “The results of this report clearly reflect our commitment and provide insight on areas where we can advance care practices.”
Among its high quality ratings, the medical group scored near the top of all ACOs for preventative health measures including influenza immunizations and screenings for high blood pressure, which, if uncontrolled, can contribute to many other health concerns. These ratings underscore PCMG’s commitment to provide information and resources to patients to prevent more serious health issues down the road.
Additionally, the medical group’s scores for medication reconciliation and fall risk screenings were among the best of all ACOs. These ratings recognize the medical group’s diligence to follow up with patients to ensure the right medications and dosages are being taken, and that patients at risk of falling take precautions to avoid serious injury.
All ACOs participating in the shared savings program are required to meet quality performance standards annually. PCMG met all of the program’s 33 quality measures, spanning the areas of patient/caregiver experience, care coordination/patient safety, preventative health and at-risk population treatment.
